Since it is located on the campus of The Heritage at Brentwood, Somerfield residents enjoy pleasant surroundings, comfortably designed suites, an enjoyable dining experience and attention to detailed, quality living. Somerfield is dedicated to a philosophy that provides services in a way that enhances personalized care based on individual choice.

Convenient Location
Somerfield is centrally located in Williamson County, right off I-65, Concord Road exit. We are located near all the major hospitals. This provides easy access to physicians’ offices for follow-up appointments or to take advantage of Somerfield’s Out-Patient Therapy programs. Situated just off Concord Road on Heritage Way near the Williamson County Indoor Sports complex, this convenient location makes it easy for friends and family to visit.
Out-patient Therapy
When those participating in Physical, Occupational or Speech therapy are well enough to return home but not quite finished with the full course of therapy, Out-patient Therapy at Somerfield provides an invaluable continuity of care with the same therapists and same individualized program. Our rehabilitative services program is focused on implementing a personalized care program to facilitate residents with their recovery and improve their well-being.
The Ultimate R&R Program
Somerfield’s Ultimate Recuperate & Return (R&R) Home Program provides targeted restorative therapy in a comfortable, residential setting. Patients can recover and return home quickly following surgeries such as hip, knee or other joint replacement. Reservations are encouraged to coordinate with your scheduled surgery date.
Respite Care
When caregivers need to be away for reasons such as work, vacation or family affairs, Somerfield can offer their loved ones a care-filled, supportive environment for as long as needed. While they are away, caregivers can have peace of mind that their loved one is in good hands and respite residents can enjoy all the services and activities of Somerfield in an atmosphere of acceptance and friendship.
Hospice Care
Somerfield provides a tender and supportive environment for terminally ill residents and their families. Every effort is made to provide as much comfort as possible. A trained and caring staff attends to resident needs and provides pain management.
Medical Director
Somerfield’s Medical Director is Richard W. Garman, Jr., MD. He makes regular rounds and is contacted for any change in resident condition. The family representative is also kept updated. Dr. Garman is certified by the American Board of Internal Medicine with an added qualification in Geriatric Medicine. After receiving his medical degree from the University of Louisville in 1980, Dr. Garman completed an internship and residency in Internal Medicine at Vanderbilt University Medical Center. Dr. Garman is a clinical instructor for the University of Tennessee and is an Assistant Clinical Professor at Vanderbilt University Medical Center. He is on staff at Baptist Hospital. Dr. Garman holds professional memberships in the Tennessee Medical Association, American Medical Association, Davidson County Medical Society, American College of Physicians, and the American Mecial Directors Association. Marathon running, cooking, and hockey are some of his favorite pastimes, and he is Team Internist for the Nashville Predators.
